By registering for and participating in this training, I agree to the following:

Attendance Requirements

Students desiring to complete the online IIQTC Level 2 Tai Chi Teacher Training and receive certification must attend the 44 hours of required Zoom training with the IIQTC Faculty.

If you miss a required training session in any of the 4 modules of instruction, we offer a make up session with one of the Faculty for a fee of $120.00 e.g.if you miss training sessions for Module 1 and Module 4, you must attend a make up session for each of the missed modules.

Program Cancellation (By Participant) Policy:

Training to prepare future teachers is a commitment we take seriously, and we trust that you take it seriously as well. It takes time, energy, and staff resources to set up and organize the details pertinent to a training event at IIQTC Headquarters. At the same time, it takes energy to organize and plan the details of your life to participate in and complete your training.

So, in honor of the energy that is required on both sides, before you click the registration button and pay the course fee, we ask that you fully understand the time commitment. We understand that life challenges may arise so if they do, inform us as soon as possible about any changes in your decision to pursue training and certification.

We sincerely appreciate your willingness and cooperation in these matters. Cancellation requests must be made to in writing to email address iiqtc@healthaction.net

If there is an extraneous circumstance where you cannot attend the program sessions, please reach out to us by email and we will do our best to assist with enrollment with future program options. If funds are transferred to another program, an administration fee of $150 will be charged to enroll. If someone cancels mid-event, they will need to pay the refresher fee for the next online event they choose.

Program Cancellation (By Health Action Inc) Policy

Health Action Inc dba Institute of Integral Qigong and Tai Chi (IIQTC) reserves the right to cancel any program, at any time. Should it be necessary to cancel a program, registrants will receive a full refund if a satisfactory alternative is not possible.

Training Waiver

I understand that the program may involve physical, mental, intellectual, and emotional activity, including exercises that may tax my physical, intellectual, mental and emotional capacity. I understand and acknowledge that my participation in the program and in every separate part of it is purely voluntary.

  • I acknowledge that my choices and my actions may pose a risk of injury to myself and/or possibly others.
  • By this consent I knowingly and voluntarily assume the risks and consequences of injury either to myself or caused to others by me in the program.

I release the Institute of Integral Qigong and Tai Chi (IIQTC), Co-Producer, IIQTC Faculty and Assistants, Health Action Inc., from any and all liability for injuries to myself. I agree to hold harmless the Institute of Integral Qigong and Tai Chi (IIQTC), Co-Producer, IIQTC Faculty and Assistants, Health Action Inc., and any of their companies from any and all liability for injuries to myself and for any injury to others caused by me.

Photo / Video Release

I hereby grant permission to Health Action Inc and the Institute of Integral Qigong and Tai Chi (IIQTC), the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video tape distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. 

Photographic, audio or video recordings may be used for ANY USE which may include but is not limited to:

  • Presentations;
  • Courses;
  • Online/Internet Videos;
  • Social Media;
  • News (Press);
  • Marketing Materials.

By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public educational setting.

I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only.

By agreeing to these policies -- Attendance Requirements, Cancellation Policy, Training Waiver, and Photo/Video Release -- I acknowledge that I have completely read and fully understand the above policies and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
